Before the incident
Quality Assurance Technician was taking a sample to perform a basis weight test in the laboratory. Metal template was placed on top of film sample and held down using the elevated handle to make correct sized cut. Employee picked up self-retracting knife and completed a majority of the cut around the template.
What happened
After making the majority of the cut the employee switched cutting hands and re placed their non-cutting hand on the template but not on the raised handle. At this time the employee went to finish the cut when the self-retracting knife made contact with the right thumb.
