Before the incident
On [REDACTED] a third shift employee was helping remove a sponge from the top of the mixer As the employee was ascending the ladder one of the support brackets on the ladder broke loose The employee fell approximately 10 feet to the floor injuring the employees left leg and arm upon impact The employee was asked if they needed medical assistance and the shift supervisor called CorVel and the local ER to inform them that I was sending an employee their way with fall injuries
