What happened
[REDACTED] around [REDACTED] a team member was working on induction putaways at [REDACTED] when they threw a box overhead into the conveyor and felt something fall into their eye. The team member immediately went to flush their eye and reported the incident to a trainer who then notified a supervisor. After several attempts the object was flushed out successfully.The team member declined treatment. The medical waiver form witness statement form and first report of incident questionnaire were completed. The team member returned to work. No toxicology screen was performed due to the nature of the incident.
