attempting to help a combative pt to use the restroom

Patient confused. complaining of unable to urinate. Thrashing around bed. Provider notified. states to place foley. While assisting primary RN with foley insertion. pt become greatly combative. Kicking. punching. and trying to bite staff. This RN. pt primary RN. and third RN on the unit all in the room trying to hold patient down to place in restraints. Pt. attempted to bite this RN on the arm. I then went to move my arm to refrain from getting bit. patient arm then wrapped around mine and patient punched this RN in the back of the head. Pt still continued to thrash around the bed. provider at bedside at this time. Order for [REDACTED] was given and provider assisted in holding patient down while nurse administered [REDACTED].
