I was in the living room in chair resting I heard him getting up so I went to check on Mr. [REDACTED]. He was on the floor as is custom to his routine of putting self on the floor to crawl due to vision loss.

I was trying to get the patient back into bed from the floor. He was partially sitting on the edge of the bed I had to lift him further into the bed twisting to the right when I felt a sharp pain to my lower back. Right low back pain-sharp pain instantly then pain in the side.
