What happened
[REDACTED] a stroke patient was brought to NSICU by anesthesiology from IR. The patient The patient became very agitated and EE thought that the E-Tube was not in place after her assessment. She called x-ray to come and verify placement ASAP. The ET Tube was verified to be bent. Pt was agitated and managed to move to the end of the bed. The nurse was trying to move the restraints so the patient did not self extubate and while doing so they pulled her on top of him from a squatted position next to the bed.
