I was lifting a patient

What happened

On [REDACTED] I picked up until [REDACTED] to help with staffing at night. I was up in the brain unit to help tech and to specifically help RN with patient who was in [REDACTED]. This patient came in with a CVA with L weakness in her [REDACTED] and was struggling with sleeping through the night and was known to be agitated at times. On this day the patient had not slept in over 36 hours was confused and stated she needed to go to the bathroom. [REDACTED] and I helped to her wc from the bed to help her to the bathroom. After the patient used the bathroom we assisted her to stand. I wiped the patient and [REDACTED] began to manage pt 's clothing to help pull up pt 's pants. However between weakness from CVA lack of sleep and confusion pt leaned to her left side into the bathroom wall and her head was hitting the grab bar. At this point I placed my right hand over the pt 's forehead to prevent her from further hitting her head and lifted the patient -max A-entirely with my left hand. Afte
