Transferring a patient from the wheelchair to the car on discharge

What happened

Two person assist transferring patient from wheelchair to car. They were attempting to transfer into a minivan. The plan was to stand pivot into the car. Patient grabbed the door which was not allowing the patient to move not helping her with the transfer with one hand and [REDACTED] 's right shoulder. [REDACTED] moved the wheelchair away. Now [REDACTED] has the back of the patient. Planned to pivot. patient loses strength buttocks is leaning on the car but not on the seat. [REDACTED] gets in the car to help pull patient into the car. Patient flops into the car. Then they work to get her legs in the car. Patient is enough on the seat to work to get each leg into the car and walking her hips back into the seat. Patient was able to get into the car safely. [REDACTED] did not feel a pull or defining moment during the incident that was damaging to her right shoulder [REDACTED] went to get the gait belt off and she was not able to raise her arm noticed a pain in her shoulder.
