Before the incident
Infiltrating lidocaine into a patient in the prone position for a cystectomy. After second infiltration of lidocaine as patient could feel the scalpel at an attempt to open the sebaceous cyst. patient was hyperventilating and after removal of needle the left second digit on palmar aspect received a puncture wound with gloves on and immediate bleeding.
