Before the incident
Team was working in the tray wash room prepping dirty tray to be inserted in the tray machine. Team Member was instructed to assist in cleaning the flour spill in silo #2.
What happened
TM was relieved of his position and assigned the task of cleaning a flour spill in silo #2. During the first load [REDACTED] requested my help; we lifted two barrels one at a time together. After this task [REDACTED] returned to the silo to continue working when he started experiencing pain in his hip.
