What happened
The employee punctured a tote of peanuts with his forks on the forklift. The employee backed the forklift up just enough to get the forks out of the tote. The employee then began taping the tote placing his right hand in between the tote and the fork. As he was taping the tote another employee pushed a tote into the tote the employee was working on causing his hand to get crushed in between the tote and fork.
