Email sent from [REDACTED]: Mr. [REDACTED] approached me yesterday claiming he went to his doctor and his doctor said he had a hernia. Thats what he is claiming. Mr. [REDACTED] said he probably got it from work from lifting items at his job. This is the first I was made aware of an injury so I do question the validity of this claim. I told him to see me before he left to make a report. I was walking with the health inspector and was not able to make a report at the time. HR spoke with EE on [REDACTED] and EE shared that he had gone to the doctor and was diagnosed with a hernia. EE carries sliders 10 LBS dogs 20 LBS fries 30 LBS bratwurst 20 LBS. EE stated that he provided a medical note stating that he can 't lift more than 10 LBS. When EE provided a medical note dated [REDACTED] we weren 't aware it was due to a hernia.
