What happened
[REDACTED] and the crew had just completed the tearoff and removing the old screws from the metal deck. [REDACTED] was removing the remaining debris off the deck as well. He stepped from the old membrane onto the metal deck where the cleanup was being completed. As he stepped on the deck he placed his left foot down on the last remaining rusty screw still installed in the deck. The screw punctured through his boot and slightly into the bottom of his left foot near his toes.
