What happened
Employee ([REDACTED]) was requested to observe issue of not filling. [REDACTED] opened up non-operator side safety gate and climbed into the machine. [REDACTED] had another employee ([REDACTED]) close the gate behind him. [REDACTED] then went back to working on a mold change that they were in the process of at the time. A second employee ([REDACTED]) then attempted to activate the fill guns but started the press in Semi-Automatic mode. [REDACTED] opened up the gate when they saw that the press was moving. [REDACTED] then opened up the operator side gate to stop the machine then closed it and put the press into manual mode to open the press. [REDACTED] was then assisted out of the press by [REDACTED].
