Before the incident
I was sitting by the door for observation of the patient; I saw that the patient was taking off her gown.
What happened
I called the nurse to tell her that the patient was removing her clothing. I went in to try assist the patient with putting back on her gown. She kept taking it off pouring water on the outlets on me. Then she started hitting me on my back.
