Before the incident
I was walking by the patient 's room when the patient was getting out of bed unassisted. Not knowing if the patient is a high fall risk or not I went to help the patient back in bed. I knew the patient was agitated and combative.
What happened
In an effort to keep the patient safe from falling I went to help the patient back in bed so he would not fall. The patient punched me in the head.
