ON [REDACTED] AT ABOUT [REDACTED] Mr. [REDACTED] was exiting his truck to pick up material and twisted his right knee on the west side of the building. I was contacted by Mr. [REDACTED] at [REDACTED] [REDACTED] his statement was given to me after being advised to call me. By Mrs. [REDACTED]. The incident was reported yesterday [REDACTED]. To [REDACTED] as soon as Mr. [REDACTED] got back to the [REDACTED] location in [REDACTED]. Currently Mr. [REDACTED] is waiting for a call back from Axiom medical to see where he needs to go to be seen. Axiom was notified at [REDACTED] [REDACTED]. Cameras were reviewed and Mr. [REDACTED] could be seen limping but no footage of incident.
