What happened
Employee was the driver of the auto incident that happened [REDACTED] that was work related. Employee declined medical attention shortly after the incident and was sent for drug test since he was the driver of the auto incident. Driver came in the [REDACTED] complaining about back pain and his left ring finger was swollen and in pain as well. I called axiom got on the phone with the nurse employee agreed to the at home remedy provided by axiom nurse. Axiom will call and check on employee periodically.
