What happened
Employee was involved in auto collision that happened yesterday that was work related. Safety asked if the employee needed medical attention time of the incident. Declined medical treatment at that time then went to the ER without notifying any BFS team member. This was discovered the [REDACTED] when the employee gave me a doctors note. When asked about this employee stated that he did not need medical attention at the time of the incident but then later decided to go to the ER. Employee was sent for drug test and then to be seen at Concentra. According to Doctors note from Concentra employee is to be put on light duty with occasionally lifting or pushing no more than 20 lbs occasionally. No bending stooping squatting kneeling or twisting. Doctors note will be attached.
