2:1 continuous observation with pt in room.

[REDACTED] became increasingly agitated; posturing at threatening and insulting 2:1 staff. Pt is remove from milieu but was allowed in his bedroom based on good behavior and doctors orders. [REDACTED] was also told to remain in his room but would not comply having the 2:1 staff there or us telling him to stay in his bedroom. He became more agitated due to being told the orders the doctors placed for him. He then struck the other 2:1 staff member and this writer began to CPI before other staff members came down when security code was called.
