The patient 's nurse and I went into the patient 's room because the bed alarm was going off. Second incident of the day: I walked by the patient 's room and noticed his restraint looked like it was loose or had come off so I went to go check on it.

First incident of the day: The nurse and I were trying to put the patient 's legs back in the bed and he kicked my stomach Second incident of the day: The nurse came to help me fix the patient 's restraint. The padding that protects his wrist from the restraint had come loose so I was trying to fix it. He grabbed my right hand thumb area and dug his nails into my hand. He wouldn 't let go. The nurse had to pull his hand off my hand.
