What happened
The team member was called to clear a jam in a saw he proceeded to lock out the electrical disconnect and failed to lock out the air shutoff valve before putting his head inside the carriage to remove the jammed material. When he removed the jammed material the carriage moved back to the home position and pinned the blade agist his neck.
