The facility had a power outage in the morning causing equipment resets. The broiler was one of the appliances affected by that outage. He was checking the broiler to see why it was not lit because it was needed to cook a regular menu item. This included turning the gas on and off and looking inside of the appliance.

The employee went to the broiler and turned on the propane. He then went and retrieved the butane torch used to finish hot dishes (e.g. melting cheese candy sugar etc.) He then looked into the broiler and brought the lit torch to the front of the opening at which time a fireball shot forward injuring the employee. He then turned extinguished the torch wiped his face and went to the staff bathroom to apply cold water.

He was diagnosed with first and second degree burns on his face and arms at the emergency room.

A propane bubble in front of the broiler ignited when it came in contact with the lit butane torch.
