Employee went to the large door to of the freezer and entered freezer. When she entered she slipped losing her balance and falling backwards landing on her butt and hitting her head on the concreate floor. When filling out incident investigation paperwork she was alert and was able to answer simple questions. She requested to go home and was driven home by a fellow employee who was her son.
