Before the incident
Re-rigging the spreader beam hook to the mesh to be moved.
What happened
Team Members were adjusting the rigging on the oversized material. The spreader beam was resting on the top of the material as they were moving around it. One team member was on the top of the material to reach the inside ties and the other was on the outside of the material. The team member on the outside of the material bent in to rig the ties the material overhang activated the crane controls moving the spreader beam which landed on top of the other team member left foot.
