Before the incident
During rework or a strand pack there was a tangle. Employee cut the strand while controlling both ends of the strand. Another employee was helping. Once cut employee attempted to pull on the end without realizing that the strand was under twisting tension.

Once the strand released it twisted in employee 's hands coming out of his grip and clipping his ear resulting in a laceration.
