Spreader beam on the Mi-[REDACTED] crane begun to swing towards the cab of the crane. As he attempted to steady the beam the chain on the beam hit him causing him to become unsteady. He jumped off the panel and when he landed he stepped on a wooden board causing him to twist his body and fall
