What happened
The employee was moving a trailer in to a repair bay using a forktruck w a trailer ball attached to the mast forks. The trailer has a 'quick coupler ' installed. This coupler allows for automatic latching locking the trailer on the tow vehicles ball. The quick release coupler is designed to be lifted off of the tow vehicle by pulling a cable connected to a release lever on the coupler then raising the trailer jack to lift the coupler off of the ball.
