Before the incident
Employee was adjusting a test regulator in the Sound Hut. The regulator is connected to a high pressure gaseous nitrogen GN2 system. Employee was manually turning a valve connected to the regulator with an attached muffler. Employee was wearing safety glasses and hearing protection.
What happened
The muffler blew out when the two muffler casing bolts failed. The semi rigid muffler material created flying shrapnel that caused the abrasions described below.
