What happened
Employee was emptying the scrap metal into the steel dumpster by raising the steel hopper to the correct height and then pushing up on the release handle which allows the hopper to tilt forward. The hopper did not tilt forward enough to empty the scrap steel. As the employee attempted to rectify the problem he failed to follow the correct safety procedures.
