Before the incident
EE was adding lime to lime vat at one of the water plants. EE had a bag of lime sitting on the ground in front of them attempting to open the top of bag with hooked box cutter.
What happened
When opening bag the hook became embedded in the plastic inner liner of the bag. The outside of the bag is made of paper but there is a plastic liner that serves as a moisture barrier. In order to cut through the plastic EE applied more force to the blade which caused the blade to cut through the plastic and exit the bag cutting EE on right thigh above right knee.
