What happened
[REDACTED] was selecting in dry PIR. He was driving the PIR machine down the aisle toward the dock. A selector had parked a triple jack in the wrong spot and the forks extended under the racking into the PIR aisle. [REDACTED] had a full rack and did not see the forks extended into the aisle. He struck the forks of the jack with the PIR machine which caused him to be launched into the PIR rack and he struck his elbow.
