Driver arrived at delivery location [[REDACTED] [REDACTED]] as stop 2 on [REDACTED]. The ambient temp was 46 deg F cloudy with heavy rain. Driver backed tractor 100805 into customer parking lot deployed ramp from the rear of 36 ' trailer 36123. Customer location was position on the driver side of the vehicle. Driver loaded wheeler and begun delivery to customer which required descending five concrete steps in good repair to enter business. On one of the trips with a loaded wheeler (2 cs of 6-10 cans 3 cs Bubly water 1 bag of panko and 1 cs of milk single stacked) the driver lost his footing on step 3 of 5 slipped and fell onto his back.
