What happened

S Phase I operator reported that he was struck on the top of the upper right shoulder O The investigation determined the associate was struck when the adjacent doff tray to the position being strung up N7 Pos 4 caught the hand held aspirator hose and pulled it out from under the hand of another operator that was stinging up the position The associate who was struck was bent down placing cones on the winder chuck when the hose was pulled and yanked over his shoulder A The investigating supervisor noticed a red spot on top of the associates right lower neck An ice pack was placed on the affected area Maintenance was called to inspect the winder and aspirator hose and to measure the length of the aspirator hose The hose was found to be within spec 14 foot and the winder was in good working condition P This incident was added to the Phase I Tier board so it could be shared across all 4 shifts This incident will be discussed during the next FDB meeting
