What happened
The employee finished the day shift as a part-time vessel-crane operator for LGC Redwing which was actively unloading a grain ship on [REDACTED]. He climbed down the crane 's interior pedestal and was walking towards the gangway. The ship had corrugated metal steps (approximately 1ft high) for crew to walk over the permanent piping running the length of the deck: this is a common feature of bulk cargo ships. At approximately [REDACTED] the employee was stepping down from one of these steps and slipped on grain chaff that had covered much of the portside deck of the ship twisting his left ankle (front top of ankle) as he slipped. He bruised the back of his left shoulder (at the shoulder blade) as well when he fell on a flat part of the deck. No LGC employees or ship crew directly observed the incident. Two supervisors and a longshoreman helped the employee down the gangway had him sit in the site buggy and alerted the terminal manager. The terminal manager called the EHS coordinator who
