The employee was entering the hold of a vessel via the integrated spiral staircase.

What happened

The employee was entering the hold via the integrated spiral staircase at the [REDACTED] of [REDACTED] ([REDACTED]). The crew of the ship were in the process of cleaning the staircase while he was descending. It was dark and there was still material at the base of the staircase where it sloped down to the material remaining in the hold. The employee was stepping from the material still lodged in the staircase to the treads of the excavator when he slipped and fell. The fall was from the distance from the material floor of the hold to where he was stepping and this was approximately three feet. Three points of contact were not established during this transition from the staircase pile to the excavator. He fell into the material at the base of the hold and dislocated his left shoulder. No one witnessed the fall. The employee was lifted out of the hold and taken to the hospital by a coworker.
