Before the incident
TM bypassed a lock out switch turned on a 4-inch diameter wheel auger on the bottom chute of the bag house and tried to clear a cement dust blockage with his right hand at which point his fingertips were caught and cut off.
What happened
3 TMs were on top of the baghouse changing out bags and dropping bags into the lower collector. 2 TMS were on the ground pulling bags out of the lower collector. The bags were getting hung up so TM came down to assist. Unrelated to the hung up bags TM bypassed a lock out switch turned on a 4-inch
