Before the incident
The employee along with another crewmember was installing a replacement hose between a compressed nitrogen cylinder and an electronic regulator manifold.
What happened
The employees collected and installed a hose without a sufficient pressure rating to the nitrogen cylinder and when the valve on the cylinder was opened the newly installed hose exploded causing temporary hearing impairment tinnitus and a severe headache to the employee.
