What happened
There are two bolts that fasten the wheel shaft to the yoke. He loosened the first bolt and leaned in to remove the bolt on the other side. While doing so placed his left hand on the yoke and began removing the second bolt. Once the second bolt came out the wheel dropped approximately 1 pinching his left pinky finger between the yoke and inner ring of the wheel. He thought he had just loosened the first bolt to keep the wheel supported but must have had it completely unthreaded.
