What happened
employee was riding on double jack -accidentally accelerated forward while at the same time turning hard. This throttle acceleration forward while turning caused employee to lose balance as the equipment accelerated away from his body . This sudden change caused employee to take foot off the platform while still accelerating toward racking structure -the walking rider made impact with racking upright with employee left foot caught in between the upright and walking rider
