Operator saw the jet didn 't look right and noticed the entry chamber wasn 't dropping. Operator shut the system down but the level chamber didn 't drop. Operator locked system out and notified coordinator about need for maintenance to help remove the valve. With the tail line and entry chamber full and unable to drain the operator instructed the maintenance team to crack the flanges of the 4 valve and let the hot liquid slowly drain to the floor.

The level in the entry chamber then proceeded to empty out. At some point the 4 valve was removed and the full 4 pipe was exposed. It is currently not known when and how full the system was when the valve was removed. The individual that was burned was around the 4 pipe when it broke free and splashed the back of his legs. The level in the entry chamber then proceeded to empty out.
