Doing the process of re-threading B2 leach tanks from a wrap of medial caught in the rollers.
What happened
The Associate slipped while picking up scrap membrane from the floor. 2 Associates were working on the tank end near LT 3 the Associate was finishing up the process of picking up membrane and filling up the garbage can when they slipped and fell hard to the ground. Associate stated they did not feel any pain and continued with the job at hand. The Associates finished with the clean up process made relief with the incoming operator and escalated the Near Miss to the 2nd Shift Supervisor in the office. A written statement was made and the Associate and Supervisor signed it and provided it to the Belts VSL.
