What happened
Finished collection blood sample and closed 25G butterfly needle (even heard it snap). I placed the needle with the attached syringe to the bottom part of the patient 's bed and finished wrapping up the patient 's site of collection. After this I grabbed the syringe with the butterfly needle attached and felt a scratch on my left forearm. I turned to look at the needle and it was not closed all the way (a small piece of the needle was outside the plastic that we pull to lock the needle). Needle was not protected all the way the bevel was exposed.
