Before the incident
Employee was assisting in removing forks off of a powered industrial truck (PIT). This task is outside the scope of the EE 's job description.
What happened
While the employee was assisting in removing forks off a PIT one of the forks crushed the employee’s left middle fingertip. The employee stated that once the fork released off the rail mast it suddenly dropped. This sudden drop did not allow the employee to remove their finger from the area thus causing the finger to be smashed in between the ground and the fork itself. In the event of his finger being smashed the damage caused was significant enough where professional medical attention was needed to repair the wound.
