What happened
Person being shown how to clear a film [REDACTED] as part of Line Leader training. Person had two instructors with him and both of them told him not to put his hand in the machine. He did it anyway and was injured by the blade on the machine. Original information taken by [REDACTED] which is who he worked for. Not until [REDACTED] that OSHA said we also needed to maintain copies of this information for reporting due to new reporting standards.
