Before the incident
On [REDACTED] a night shift employee noticed high water in the caustic tank secondary containment. To prevent pump damage they activated and activated the area sump.
What happened
The employee was not aware the liquid was a caustic rain-water mix. Liquid transferred from the caustic containment to the [REDACTED] secondary containment drained to the [REDACTED] 2 sump area and was pumped into the recycle tank. The caustic caused the pH to spike which caused the system to foam over. To stop the unit from foaming and overfilling the secondary containment employes used five-gallon buckets to transfer defoamer into the tank. During this process one employee was exposed to steam smoke coming out of the tank.
