Unloading a truck

Employee was working with [REDACTED] to unload a truck a long pallet with a poorly stacked load that was blocking the dock plate from functioning. While manually unloading a heavy stuck item suddenly shifted causing [REDACTED] to lose balance and fall with his foot wedged into the gap between the truck and dock.
