Picking cases to a conveyor.

What happened

The employee was picking up cases off a pallet and placing them on a table. (the cases weight about 35 lb’s) The area was very tight and congested because somebody placed 4 pallets within 12” of the table and the 4 pallets were pushed tight up against each other so there was no space to maneuver between them. As the employee got to the bottom layer of the pallet she reached across the pallet to pick up the 2nd last case as she was standing up because her feet were in a tight space she lost her balance and fell backwards with the box in her hand. She did not fall to the floor she was able to catch herself with her feet and her body weight was leaning backwards she got her feet in a kind of crouching position to prevent from falling on her back. She then stood up and placed the box on the table. Then went towards the pallet and tried to move it so it didn’t happen again for the last box.
