What happened
EMPLOYEE INSERTED A PART INTO THE LATHE CHUCK TO BURNISH. HE MANUALLY TURNED THE PART WITH THE SAFETY SHIELD DOWN TO ENSURE THERE WAS NO ISSUE. AFTER THAT HE POWERED UP MOVING SLOWLY. HE TURNED TO GET HIS SCOTCH PAD AND WHEN HE APPROACHED THE SPINNING PART THE SHIELD WAS STRUCK BY THE LATHE HEAD WHICH BROKE AND STRUCK HIS LEFT HAND.RESULTED IN BEING TREATED OFF-SITE FOR LACERATION AND BRUISING. RECEIVED STITCHES.
