What happened
Forklift driver was placing bins down on the floor in front of the operator. There is a metal angle iron on the floor to prevent the bins from being placed too close to the operators. Forklift driver bumped the angle iron. 2 of the bolts holding the angle iron to the floor were broken so when the driver bumped the angle iron it allowed the angle iron to pivot. When it pivoted it hit the employees foot.
